End of Life Care strategy (2008)

 

 

The Department of Health has released their national 'End of Life Care Strategy - promoting high quality care for all adults at the end of life'.

 

Implementation of this strategy will make a step change in access to high quality care for all people approaching the end of life. This should be irrespective of age, gender, ethnicity, religious belief, disability, sexual orientation, diagnosis or socioeconomic depravation. High quality care should be available wherever the person may be: at home, in a care home, in hospital, in a hospice or elsewhere.

NHS Nottingham City has developed a questionnaire to find out more about what we all think about death and dying. The answers to this questionnaire will help to make local services more responsive.

 

The questionnaire is available to download here. Once you've filled it in you can email it to pals@nottinghamcity.nhs.uk or print it and return it to the following freepost address:
