Care Plans
Anyone who provides structured interventions to people with drug/alcohol problems is required to deliver treatment as part of a structured care plan.
The care plan is used to identify both the drug/alcohol treatment plan and the individual's wider health and social needs.
A care plan should put the patient/client at the centre of the planning process. It should be a living document that highlights options and opportunities available at any given time on the treatment pathway.
